Federal Verification Forms

The forms for the 2017-2018 year are below.  Select the forms for the academic year that we have requested.  Please scroll down to access the forms for both academic years. 

IMPORTANT: Please only complete the following requirements if specifically requested by the Financial Aid Office.

For your convenience, electronic documents can be securely submitted using our Secure Document Upload application.

2015 IRS Tax Return Transcripts

PLEASE DO NOT SEND COPIES OF YOUR 1040 FORMS. We cannot accept the 1040 form, we require the official IRS Tax Return transcript.

If you have not done so, you can complete this requirement by using the IRS Data Retieval Tool (IRS DRT) which is a part of FAFSA on the Web at https://fafsa.ed.gov.

Otherwise, to obtain a 2015 IRS Tax Return Transcript, go to www.irs.gov/Individuals/Get-Transcript and complete the request process.  Make sure to request the "Return Transcript" and NOT the "Account Transcript".  Only submit if requested. 

Household Information Form Please complete and submit the Federal Verification Household Information form for 2017-2018 only if requested.
2015 W-2 Forms Please submit a copy of W-2 Form(s) only if requested.
Student Nontax Filer Statement
On the FAFSA, you indicated that the student was a non-tax filer in 2015.  Please complete and submit the Federal Verification Student Nontax filer statement for 2017-2018, only if requested.
Parent Nontax Filer Statement On the FAFSA, you indicated that the parent(s) are non-tax filers in 2015.  Please complete and submit the Federal Verification Parent Nontax filer statement for 2017-2018, only if requested.
Federal Reject, review your SAR for the resolution needed If you have received this notice, your financial aid cannot be processed due to a Federal Student Aid Report (SAR) reject issue.  Please contact our office for help and additional information if necessary.
FAFSA issue to resolve: Selective Service The Selective Service reported that you have not registered with them.  If you are female or were born before 1960, registration is not required.  Otherwise, if you are not yet registered, are male, and are 18 through 25 years of age, to receive aid you must do one of the following: 1) Register online at www.sss.gov, or 2) Request registration on your Student Aid report by answering "Male" to item 21 and "Register Me" to item 22, or 3) Complete a Selective Service registration form at your local post office.  If you have documentation proving an exemption, submit the documentation to the Financial Aid Office.
FAFSA issue to resolve... If you receive a notice of this type, please contact the Financial Aid Office for assistance in resolving the FAFSA issue.
2017-2018 Duquesne Financial Aid Application Please complete the Duquesne University Financial Aid Application if requested.  This application is required if you plan to attend part-time.
